    SUMMARY Psychological aspects in the evaluation of the diabetic patient treated with infusion pump-therapy: current state and perspectives This article aim is to offer a close examination of up-to-date theoretical contributions referring to the following subject: psychological aspects in CSII therapy. This close examination was made considering two different researches: the first by regarding the evaluation of the post implant period, and the second concerning the study of drop out causes. It was possible to find out problems and psychosocial changes caused by the implant, but also to specify critical points, useful to build necessary protocols, to support diabetological valuations and to suggest psychoeducational interventions

    Lymphedema is an important medical issue around the world, caused by an anomalous collection of fluid in soft tissue due to congenital malformations or stenosis or obstruction of lymphatic vessels. Magnetic resonance lymphangiography (MRL) is an emerging technique focused on noninvasive or minimally invasive imaging of lymphatics with the goal to diagnose and treat lymphedema. This review will briefly discuss lymphatic imaging starting with lymphography and radionuclide lymphoscintigraphy up to the newest methods, focusing on MRL, a rising technique, and highlighting the technical aspects fundamental for achieving high-resolution MRL

    SUMMARY Psychological adjustment to type 2 diabetes Objective. Based upon the literature of the past ten years, this study analyzes the initial phase of psychological adjustment after a diagnosis of type 2 diabetes, with the goal of providing useful guidelines to diabetologists in supporting their patients from the very first moment of clinical work. The study identifies five functional areas that characterize this adjustment: anxiety, depression, locus of control, self-esteem and self-efficacy. Method. Using a battery of psychometric tests, we assessed the psychological condition in a sample of 42 subjects with type 2 diabetes, comparing with a control sample of 420 subjects, without diabetes. Results. The initial psychological adjustment after six months from diagnosis, seems characterized by decreased tone of mood and self-efficacy, without anxious symptoms. The subjects seem to develop a fatalistic attitude or a strong external entrustment, showing inclination to underestimate the disease’s reality and to seek external dependence that could affect the path towards an adequate and effective self-management. Conclusions. The authors discuss the findings emerged and possible directions for a smoother working relationship between diabetologists and their patients

    Towards ecological flows: status of the benthic macroinvertebrate community during summer low-flow periods in a regulated lowland river

    Climate change along with the increasing exploitation of water resources exacerbates low-flow periods, causing detrimental effects on riverine communities. The main mitigation measure currently adopted to counteract hydrological alterations induced by off-stream diversion is the release of minimum flows (MFs), even if within the European Union Water Framework Directive an upgrade towards ecological flows is urgently required to achieve good ecological status (GES). In this study, we investigated the temporal evolution of the benthic macroinvertebrate community in an Italian-regulated lowland river (Ticino River) to clarify the ecological effects of summer low flows, and we evaluated the current MFs in the perspective of meeting GES standard. Biomonitoring was carried out for four consecutive years (2019-2022), in a river site immediately below a large off-stream diversion. The four study years were characterized by different streamflow patterns, thus allowing us to compare the temporal trajectories of the community under different flow conditions. Moreover, the interruption of the low-flow periods due to overflow spilled by the upstream dam gave us the opportunity to assess the effects of experimental flow peaks. Contrary to the expectation, the macroinvertebrate assemblage kept almost unvaried across the years, showing great resistance and resilience to hydrological changes. Even in extraordinarily dry 2022, the community composition varied only slightly, with a reduction of mayflies and an increase of mollusks. However, a deterioration of the ecological status below GES standard was recorded that summer, indicating the need for an upgrading of the current MFs. This upgrade would include experimental flow peaks in critical periods, which act as intermediate disturbances, enhancing community richness, diversity, and overall quality, as well as compliance with a threshold of an index specifically developed for the hydrological pressure

    Investigating the Paracrine Role of Perinatal Derivatives: Human Amniotic Fluid Stem Cell-Extracellular Vesicles Show Promising Transient Potential for Cardiomyocyte Renewal

    Cardiomyocyte renewal represents an unmet clinical need for cardiac regeneration. Stem cell paracrine therapy has attracted increasing attention to resurge rescue mechanisms within the heart. We previously characterized the paracrine effects that human amniotic fluid-derived stem cells (hAFSC) can exert to provide cardioprotection and enhance cardiac repair in preclinical models of myocardial ischemia and cardiotoxicity. Here, we analyze whether hAFSC secretome formulations, namely, hAFSC conditioned medium (hAFSC-CM) over extracellular vesicles (hAFSC-EVs) separated from it, can induce cardiomyocyte renewal. c-KIT+ hAFSC were obtained by leftover samples of II trimester prenatal amniocentesis (fetal hAFSC) and from clinical waste III trimester amniotic fluid during scheduled C-section procedures (perinatal hAFSC). hAFSC were primed under 1% O2 to enrich hAFSC-CM and EVs with cardioactive factors. Neonatal mouse ventricular cardiomyocytes (mNVCM) were isolated from cardiac tissue of R26pFUCCI2 mice with cell cycle fluorescent tagging by mutually exclusive nuclear signal. mNVCM were stimulated by fetal versus perinatal hAFSC-CM and hAFSC-EVs to identify the most promising formulation for in vivo assessment in a R26pFUCCI2 neonatal mouse model of myocardial infarction (MI) via intraperitoneal delivery. While the perinatal hAFSC secretome did not provide any significant cardiogenic effect, fetal hAFSC-EVs significantly sustained mNVCM transition from S to M phase by 2-fold, while triggering cytokinesis by 4.5-fold over vehicle-treated cells. Treated mNVCM showed disorganized expression of cardiac alpha-actinin, suggesting cytoskeletal re-arrangements prior to cell renewal, with a 40% significant downregulation of Cofilin-2 and a positive trend of polymerized F-Actin. Fetal hAFSC-EVs increased cardiomyocyte cell cycle progression by 1.8-fold in the 4-day-old neonatal left ventricle myocardium short term after MI; however, such effect was lost at the later stage. Fetal hAFSC-EVs were enriched with a short isoform of Agrin, a mediator of neonatal heart regeneration acting by YAP-related signaling; yet in vitro application of YAP inhibitor verteporfin partially affected EV paracrine stimulation on mNVCM. EVs secreted by developmentally juvenile fetal hAFSC can support cardiomyocyte renewal to some extension, via intercellular conveyance of candidates possibly involving Agrin in combination with other factors. These perinatal derivative promising cardiogenic effects need further investigation to define their specific mechanism of action and enhance their potential translation into therapeutic opportunity

    Structured and shared CT radiological report of gastric cancer: a consensus proposal by the Italian Research Group for Gastric Cancer (GIRCG) and the Italian Society of Medical and Interventional Radiology (SIRM)

    Objectives Written radiological report remains the most important means of communication between radiologist and referring medical/surgical doctor, even though CT reports are frequently just descriptive, unclear, and unstructured. The Italian Society of Medical and Interventional Radiology (SIRM) and the Italian Research Group for Gastric Cancer (GIRCG) promoted a critical shared discussion between 10 skilled radiologists and 10 surgical oncologists, by means of multi-round consensus-building Delphi survey, to develop a structured reporting template for CT of GC patients. Methods Twenty-four items were organized according to the broad categories of a structured report as suggested by the European Society of Radiology (clinical referral, technique, findings, conclusion, and advice) and grouped into three "CT report sections" depending on the diagnostic phase of the radiological assessment for the oncologic patient (staging, restaging, and follow-up). Results In the final round, 23 out of 24 items obtained agreement ( >= 8) and consensus ( 0.05). Conclusions The structured report obtained, shared by surgical and medical oncologists and radiologists, allows an appropriate, clearer, and focused CT report essential to high-quality patient care in GC, avoiding the exclusion of key radiological information useful for multidisciplinary decision-making
